Grants
The John Mondati Foundation supports programs and initiatives that promote an integrated approach to healing and empowering patients to take responsibility for their health. Grants will only be provided to 501(c) (3) non-profit organizations. Our grant making is generally limited to organizations in the greater Twin Cities area of Minnesota.
The Application Process
If your organization is interested in submitting a grant proposal, click the link below to download a copy of the Minnesota Common Grant Application along with additional information required for consideration. Proposals should be received by September 1 for review at the Fall meeting or March 1 for review at the Spring meeting.
